Apply for a duplicate through a WV DMV regional office. Walk-ins are accepted, and appointments are also available if you prefer a scheduled time.
Do not use the online duplicate-registration options for a driver credential; those services are for vehicle records. WV DMV’s driver-credential duplicate guidance points applicants to the duplicate document requirements and regional-office process. Unless DMV gives you a permit-specific remote option for your case, plan on the regional office route.
A police report is not listed as a standard document requirement on WV DMV’s published duplicate credential pages. You can still report the theft if identity misuse or another crime is a concern. For DMV replacement, follow the standard or REAL ID duplicate-document rules that apply to your permit.
WV DMV’s public duplicate pages do not spell out a separate parent-or-guardian rule for every minor duplicate transaction. Because Level 1 permits are issued to minors under the GDL system, confirm guardian participation or consent with DMV before traveling if the permit holder is under 18.
Contact WV DMV before trying to drive or making a special trip. The public duplicate pages do not publish a general mail or online replacement path for an out-of-state instruction-permit holder. West Virginia requires the permit to be in the driver’s possession while driving, so wait for DMV’s valid replacement or temporary-document instructions.
Current duplicate document rules are at https://dmv.wv.gov/not-federal-identification-card-requirements and https://dmv.wv.gov/federal-identification-real-id-card-requirements.
You do not automatically repeat the entire original application document set for every duplicate.
| Replacement Type | Document Approach |
|---|---|
| Standard / Not for Federal Use duplicate | WV DMV lists one proof of identity and one proof of West Virginia residency, with additional documents when a name or address change applies. |
| REAL ID duplicate | REAL ID replacement rules are stricter. Use the current WV DMV REAL ID duplicate requirements for the exact documents. |
A WV-employer paycheck stub showing the current physical address can be an accepted residency document when it meets DMV requirements. A properly performed out-of-state notarization is not automatically invalid.
If the permit is damaged or unreadable, request a duplicate so you have a usable credential. Keep this page focused on the replacement transaction rather than treating a damaged barcode or card as a separate permit category.
West Virginia law requires the instruction permit to be in the driver’s immediate possession while driving. A phone photo or screenshot is not the same as an official credential. West Virginia Mobile ID exists, but current DMV material does not clearly say that it replaces the physical-permit possession requirement for permit driving.
If DMV issues a valid temporary permit document during the duplicate transaction, follow the restrictions shown on that document while waiting for the replacement card.
"The AI mock tests were surprisingly realistic. The explanations for road signs helped me understand the logic, not just memorize. Passed my permit test on the first try!"

"I was struggling with the specific road rules of my state until I used Shoro. The flashcards are a game changer for quick revision before the actual exam."
