The current Wyoming credential application is Form 10-4003 R05/25, the Driver License / Identification Card Application. It is used for instruction permits, driver licenses, and identification cards.
You can download the current Form 10-4003 R05/25 Driver License / Identification Card Application from WYDOT. Paper copies are also available at Wyoming Driver Services exam stations.
How long can you keep a completed Form 10-4003 before submitting it? The current Form 10-4003 does not print a separate expiration period for a completed first-time application. Use the current R05/25 version and complete a fresh form if your address, legal name, medical answers, or other information changes before you submit it.
Form 10-4003 R05/25 asks for your identifying, residency, eligibility, and signature information.
Full Wyoming permit rules are at the Wyoming learner's permit guide.
Applicants under 18 should follow the parent or legal-guardian signature requirements shown on the current form and bring the adult's identification if WYDOT requires it for the transaction.
| Part | Who Handles It |
|---|---|
| Applicant information | The applicant provides their legal, address, and eligibility information. |
| Applicant signature | The applicant signs where required. |
| Minor consent | A parent or legal guardian completes the required consent/signature portion for an applicant under 18. |
| No-SSN affidavit | An applicant who has never been issued an SSN and qualifies for this path signs the sworn declaration. |
Can you electronically sign the Wyoming permit application? For an in-person permit transaction, do not assume a typed or pasted electronic signature will be accepted. Sign the application as Driver Services directs at the transaction, or use the signature method provided inside an official oneWYO workflow when that service is available for the task.
The standard Form 10-4003 does not include a general notarization requirement for the applicant signature. Minors still need the required parent or guardian certification. Do not confuse this with Wyoming's separate restricted hardship application, which does require notarization.
Enter the residential address where you actually live in the residential-address fields. If you receive mail somewhere else, use the separate mailing-address fields. Your Wyoming residency documents must support your physical residential address, not only a PO Box or mail drop.
| Document Type | Requirements |
|---|---|
| Proof of Identity | Original or certified birth certificate, valid U.S. passport |
| Social Security Number | Provide the SSN if issued; proof is recommended, not required |
| Two Wyoming Residency Proofs | No more than 45 days old and from different entities |
| Completed Form 10-4003 R05/25 | Complete the applicable sections and required signatures on the current form |
| Parent/Guardian Signature | Required for applicants under 18 under the Legal Guardian Certificate rules |
| Application Fee | $45 |
If you spot an error, the solution depends on when you catch it.
If the issued permit itself contains wrong information, use the dedicated permit-error correction guide for the correction process and required documents. This application-form page only covers mistakes caught while completing or submitting the form.
After the form is accepted, the standard permit process continues with the required screening and knowledge test. Processing and card-delivery timing are covered in the dedicated processing-time guide.
A Wyoming instruction permit is valid for 1 year.
"The AI mock tests were surprisingly realistic. The explanations for road signs helped me understand the logic, not just memorize. Passed my permit test on the first try!"
