Washington doesn't give you a single paper form to fill out. The state uses an online pre-application system that generates your permanent WDL number required on every Washington instruction permit application form. One paper form: the official form: is required only for minors needing parental consent.
The online pre-application starts your record and generates the WDL number. It does not complete the permit transaction by itself; the remaining permit requirements are finished through the applicable DOL process. The Parental Authorization Affidavit is a separate paper form used when required for a minor.
DOL describes the WDL number as your unique 12-digit Washington driver-license, ID, or permit number and does not publish a separate expiration date for the number itself. If you already have a Washington license, permit, or ID record, do not create a new pre-application just to get another WDL number.
Check an existing Washington license, permit, or ID card and your License Express account first. If you still cannot locate the number, contact DOL rather than creating a second pre-application record.
Start with the online pre-application. Enter your legal name and other identifying information exactly as it appears on your source documents, then save the WDL number the system creates.
Yes. Applicants under 18 must have parental consent. Parents sign the official form to authorize the permit.
Parents assume financial liability for the minor's driving. In divorce situations, the custodial parent signs.
Washington's PARENT ARRIVED LATE rule has real consequences. At the Bellevue DOL, King County Metro ran short of the stop: Mom arrived 18 minutes late. Slot cancelled; joint rescheduling took 7 days. King County Metro short-turns happen regularly on Route 271: take the earlier bus to Bellevue DOL.
Fixing errors depends on when you catch them. Corrections are free before you pay: after that, expect a $15 replacement fee.
| When Error Found | What To Do | Cost |
|---|---|---|
| Online Pre-Application | Log into License eXpress and edit your profile | $0 |
| At DOL Counter (Before Payment) | Tell the clerk to update data on screen | $0 |
| After Permit Issuance (Address/Description) | Update online via License eXpress and order replacement | $15 |
| After Permit Issuance (Name/DOB/Gender) | Visit DOL with original proof documents | $15 |
A mismatch between your permit and ID will get you turned away from the skills test. You'll forfeit your test fee. Incorrect data also transfers to your full license if uncorrected.
Do not keep using both records. Washington DOL tells people who already have a Washington license, permit, or ID record not to pre-apply again. Contact DOL so staff can identify which record you should use for the permit transaction.
DOL does not publish a self-service edit process for every field after pre-application. If something is wrong, correct what License Express allows and tell DOL about the remaining error before the permit is issued instead of submitting another pre-application.
Form-specific problems include a name or birth date that does not match your identity record, an incorrect WDL number, a missing required signature, or an outdated authorization form.
Check the online confirmation before the appointment and compare your identifying details with the documents you will use. Broader document-rejection issues belong in the required-documents guide.
The digital pre-application is mandatory. the official form is the only paper form that matters.
Complete the online pre-application at least one week before your desired appointment date. This gives you time to catch any data entry errors and ensures your WDL number is fully active in the system. Review the Washington Driver Guide while waiting for your appointment.
"The AI mock tests were surprisingly realistic. The explanations for road signs helped me understand the logic, not just memorize. Passed my permit test on the first try!"
