South Carolina uses Form 447-NC for the beginner's permit application.
The South Carolina DMV permit application form number is Form 447-NC (Application for Beginner's Permit, Driver's License, or Identification Card). Applicants under 18 must also have a parent or guardian complete Form 447-CM (Consent for Minor).
Download Form 447-NC free at dmv.sc.gov. Print it, fill it in black ink at home, but do not sign until you are in front of an SCDMV employee.
Use the current Form 447-NC from SCDMV. If the PDF allows typed entries, you can prepare those fields before printing, but bring the physical form and leave any signature that must be witnessed for the SCDMV employee.
What if you downloaded an older version of Form 447-NC? Check the SCDMV website again before your visit and use the current posted form. That avoids arriving with a version that no longer matches the active application.
Use your full legal name exactly as it appears on your birth certificate. Use a physical SC street address, not a PO Box. Answer all medical questions truthfully.
Complete every field on Form 447-NC before arriving to avoid being sent to the back of a long line at a busy branch like the one in Columbia.
A parent or guardian must sign the Authorized Adult section of Form 447-NC and Form 447-CM in front of an SCDMV employee while showing their own valid picture ID.
For applicants under 18, a parent or guardian must:
Complete South Carolina permit details are on the South Carolina permit guide.
If a parent cannot attend, they must provide a notarized letter of permission authorizing another responsible adult to sign on their behalf.
| Applicant Age | Parent Must Sign | Witness Required |
|---|---|---|
| 15–17 | Form 447-CM & Form 447-NC | Yes (SCDMV Employee) |
| 18+ | Not Required | No |
The applicant completes the personal and credential information that applies to them. For a minor, the authorized adult completes and signs the adult-consent portions required by the form and SCDMV process.
If you make a mistake on your beginner's permit application, do not use correction fluid. Draw a single line through the error, write the correct information next to it, and initial the change.
Common Form Mistakes:
Bring a backup pen to the DMV. Using the provided pens that often skip or smear can make your form look altered, causing unnecessary scrutiny and delays.
Supporting documents depend on the credential type. A standard credential needs one proof of South Carolina address; REAL ID needs two. See Required Documents for the full identity, SSN, and residency checklist.
Check your documents for exact name matches 48 hours before your visit. If you recently married, update your name with the Social Security Administration first; the Rock Hill DMV cannot process a Real ID application if the SSA database hasn't been updated.
Form 447-NC is only the application step. See the permit test guide for test details, fees and costs for current charges, and the road-test guide for holding-period and next-step rules.
Do you need a new Form 447-NC when switching from a standard credential to REAL ID? REAL ID is a different card choice on Form 447-NC. Use the current application and bring the additional REAL ID documents if SCDMV does not already have them on file.
"The AI mock tests were surprisingly realistic. The explanations for road signs helped me understand the logic, not just memorize. Passed my permit test on the first try!"
