The Rhode Island Division of Motor Vehicles (DMV) uses a single form for all learner's permit applications. The correct form number is essential to avoid delays.
The official Rhode Island DMV permit application form number is Form LI-1, titled 'Application for License, Identification Card and Permit.' This multi-purpose form is mandatory for all new drivers.
You can obtain the LI-1 form in two ways:
The completed form is valid for your DMV appointment. A Rhode Island learner's permit is valid for one year from the date of issue.
The current LI-1 tells applicants to use black or blue ink. Complete it as a paper form unless the DMV specifically directs you to do otherwise. The required applicant and parent/guardian signatures must also follow the form's signature and notarization rules.
Complete Form LI-1 in black or blue ink. Write your full legal name exactly as it appears on your identity document, enter your Social Security number, select the correct permit type, and leave no required fields blank.
For an applicant under 18, a parent or guardian signs the consent section in person at the DMV or before a notary if the parent or guardian cannot attend.
Follow these sections carefully:
For Rhode Island permit process details, see the Rhode Island permit guide.
For a first learner permit, select PERMIT and NEW in the transaction section. The LI-1 does not provide separate Limited Instruction Permit and adult Instructional Permit checkboxes; DMV applies the correct permit type from your age and transaction.
What should you write on LI-1 when a field does not apply? Sections A through D are marked "complete all fields." Do not invent information or silently skip a required field. If a field truly does not apply to you and the form does not give an option, ask DMV how it should be marked before submitting the application.
Parents can help fill out the details, but the applicant must sign the form themselves.
The parent's signature for a minor accepts financial liability under Rhode Island law.
The LI-1 is set up for handwritten applicant and parent/guardian signatures and, when required, a notary signature and stamp. Use original signatures on the printed form rather than typed names or pasted electronic signatures.
Errors cause rejections and rescheduling.
| Common Error | Consequence | How to Prevent |
|---|---|---|
| Name mismatch with birth certificate | Application rejected; need new proof documents | Copy name exactly from your passport or birth certificate |
| Parent signature not notarized (when required) | Cannot process application; must reschedule DMV visit | Get the parental consent section notarized before your appointment if the parent won't be present |
| Failing to initial corrections on the form | Clerk may consider the form altered and request a new one | Always draw a line through errors, write the fix, and add your initials |
If you find an error before submitting, cross it out with a single line, write the correction above, and initial it.
Do not use correction fluid. If you discover the error after the permit is printed with wrong information, use the permit-correction process. The current duplicate permit fee is $13.50.
If you catch a mistake before or during your DMV appointment, you can fix it for free.
If the printed permit contains incorrect information, use the dedicated permit-correction process instead of repeating the full LI-1 instructions here. A duplicate permit costs $13.50; see the Errors on Permit guidance for the correction path and supporting documents.
At the East Providence office, Third-party Rhode Island portal had the prior year's form. Rejected; current form printed at the front desk, resubmitted same day.
Only dmv.ri.gov carries the current form; third-party portals lag by months..
Maya filled out the LI-1 form correctly and brought every document on the checklist. The clerk flagged her secondary ID — it had expired two months earlier. She rescheduled and replaced it with a utility bill.
"The AI mock tests were surprisingly realistic. The explanations for road signs helped me understand the logic, not just memorize. Passed my permit test on the first try!"
