Rule	Detail
The form specifies 40 daytime and 10 nighttime minimums (split is on the form itself).
Each drive must be logged individually with date, supervisor name, Ohio license number, and start/end time.
Falsifying the affidavit is signing under penalty of perjury - keep receipts, app logs, and timestamps as backup.
The eligible supervising adult is a parent, guardian, custodian, or a licensed adult over 21 acting in loco parentis.
TIPIC holders issued on or after Sept 30, 2025 may have updated logging requirements - confirm permit issuance date against current BMV guidance.
