Form LIC100 is the official Massachusetts RMV learner's permit application. It covers Class D (car) and Class M (motorcycle) permits.
Download Massachusetts RMV Form LIC100 free from Mass.gov/rmv before your appointment. The online method is fastest and ensures you have the current version. Paper forms at the RMV office are available but can run out during busy periods. Never print from a third-party site since outdated versions get rejected at the Massachusetts RMV counter.
Fill out Massachusetts RMV Form LIC100 completely in all five sections: applicant information, license class requested, parental consent, certification, and signature. Use blue or black ink only. Do not use correction fluid. Cross out mistakes with a single line and initial them. Incomplete or altered forms trigger automatic rejection and require you to start over.
Section A: Select 'Learner's Permit' and your class (D or M). Choose REAL ID or Standard.
Section B: Enter your full legal name exactly as it appears on your birth certificate or passport. Include your Social Security Number.
Section D: List height, weight, and eye color. Organ donor registration is optional. Voter registration is now automatic.
Section G: Answer all medical and driving history questions truthfully. False answers are a criminal offense.
Section J (Under 18): Your parent or guardian must complete and sign this section. It grants permission and acknowledges liability.
Your permit is valid for two years after issue. The application information itself must be accurate at submission time.
Massachusetts's cached form version trap is easy to miss. A Cambridge applicant's browser had the previous RMV form version, updated 3 days earlier. It was rejected; the current version was printed at the branch and resubmitted within the hour. Always reload mass.gov/rmv fresh the morning of your visit.
What if you selected the wrong Class D or Class M option? The RMV application allows Class D, Class M or Both (D/M). Correct the class before the permit transaction is finalized so the exam and permit match the vehicle class you actually need.
What if you selected Standard instead of REAL ID, or REAL ID instead of Standard? Tell the RMV before the credential transaction is completed. REAL ID has different document requirements, so changing the selection may require additional proof.
Which LIC100 fields can be left blank? Leave a field blank only when the form marks it as not applicable to your transaction. For example, a current Massachusetts permit/license number is only required when you already have one. Complete the service type, applicant information and any sections that apply to your permit request.
Parents can help you fill out fields, but you must personally sign the form. Under 18? A parent or guardian must also sign Section J.
That parental signature certifies three things: permission for the permit, supervised driving hours, and financial liability for accidents.
If your parent cannot attend the RMV appointment in person, their paper signature may require notarization to avoid rejection.
| Applicant Situation | Who Signs |
|---|---|
| Age 18 or older | The applicant signs the application. |
| Under 18 with a parent or legal guardian | The applicant signs, and the parent or legal guardian completes the required consent section. |
| Under 18 when the parent or guardian cannot attend | The parent or guardian signature may need notarization before the RMV visit. |
| Under 18 with another authorized custodian | An authorized legal custodian, such as a DCF social worker or boarding-school headmaster when permitted, signs with the required proof of authority. |

Spot an error before paying? Tell the clerk for a free fix. After the permit issues, corrections cost $15. If the error is discovered after the permit is mailed, you must return to a Massachusetts RMV Service Center in person with the permit card and original identity documents. Mail-in corrections are not accepted for any permit errors.
| Error Type | How to Fix It | Cost |

*The RMV may waive this fee if it was their clerical mistake.
A Springfield applicant used his nickname on the form. The RMV clerk rejected it because it didn't match his passport. He had to reschedule his test weeks later.
Do not assume every field remains editable after submission. If you notice an error before the RMV finishes the transaction, review it in myRMV and tell the clerk at your appointment. The RMV application also has a Change of Information transaction for items such as name, address, date of birth, height and eye color.
Online forms beat paper ones every time. The myRMV process pre-fills your information, uploads documents, and checks your work. Hand-filled forms get rejected for illegible handwriting, missing info, and wrong document types.
At Worcester RMV, a teen's parent signed but forgot to print their name below the signature. The clerk couldn't process it. They waited days for a notary.
In Boston, an applicant's address got transposed digits on the clerk's screen. She didn't catch it before signing. Correcting it cost $15 and delayed her REAL ID upgrade.
Three concrete steps prevent these failures:
That last one saves you 2-3 weeks of wasted time.
For under-18 applicants with limited residency proof options, download the RMV Residency Affidavit and bring it signed to your appointment.
Parents also sign a separate liability consent form during the junior operator license road test process if your teen is taking the road exam.

"The AI mock tests were surprisingly realistic. The explanations for road signs helped me understand the logic, not just memorize. Passed my permit test on the first try!"
