You need three categories of original documents to get an Iowa instruction permit. Photocopies won't work.
You must bring identity proof, Social Security verification, two residency documents, parental consent if under 18, and $6.00.
| Category | What to Bring |
|---|---|
| Proof of Identity | Certified U.S. Birth Certificate OR Valid U.S. Passport/Passport Card |
| Proof of Social Security Number | Social Security Card (signed, not laminated) OR W-2/1099 showing full SSN |
| Proof of Iowa Residency | Two separate documents with your name and Iowa physical address |
| Parental Consent (if under 18) | Parent/guardian in person OR notarized Form 430018 |
| Payment | $6.00 (cash, check, or card with service fee) |
Use Iowa DOT's customized document checklist tool to confirm your exact requirements and download the REAL ID checklist PDF before heading to the service center.
The $6.00 permit fee is set by Iowa Code 321.191.
A certified birth certificate is the most common identity document. It must be state-issued with a raised seal or stamp.
If your birth certificate name doesn't match your current name, bring certified legal documents linking them. Marriage certificates, divorce decrees, or court-ordered name change papers work.
Download the Iowa Driver's Manual PDF to review all permit eligibility rules before your appointment.
An Iowa City applicant used a library card as their secondary document. The document check clerk declined it before the application was reviewed. Iowa requires a document from the approved secondary ID list.
Bring certified legal-name-change documents that connect the name on the identity document to your current legal name. Iowa DOT lists documents such as a certified marriage certificate or a court order when a name change must be documented. See Iowa DOT's name-change document rules.
Can an expired passport or ID be used for an Iowa permit application? Do not rely on it. Iowa's document checklist says documents must be original or certified copies and unexpired unless the checklist specifically says otherwise. A U.S. passport used as primary identity proof must be valid and unexpired.
Two separate documents proving your Iowa physical address are required. PO Boxes don't qualify, and bills must be dated within 30 days.
Minors living with extended family can use that address. Your parent or guardian must accompany you to sign a residency affidavit at the service center. You'll need your certified birth certificate to prove the relationship, and the adult provides two residency documents for their address.
Download the Minor Residency Affidavit from the Iowa DOT forms page if using a guardian's address. If last names differ, bring documentation like a marriage certificate to explain the relationship.
A parent, guardian, or custodian can document the minor's Iowa residency through the consent process, and the minor can use a second printed document showing the minor's name and Iowa address, such as a report card, medical form, sports enrollment paperwork, or mail. Iowa DOT lists the minor residency options.
Can electronic bank statements or utility bills be used as Iowa residency proof? Bring printed copies. Iowa DOT's REAL ID checklist asks for two printed residency documents, so a phone screenshot alone should not be treated as the document you plan to present.
Your document must display your full Social Security number for SSA database verification.
Nathan's Social Security card had peeling edges from years in a wallet. The Des Moines DOT clerk flagged it as a tampering risk and rejected it. A new card took 10 days from the SSA.
Unmarried applicants under 18 need written consent from a parent, guardian, or legal custodian. The signer accepts financial liability for your driving.
Divorced parents only need one signature. Typically, the custodial parent or the one you live with signs. Both parents are not required.
notarizing forms at a bank takes five minutes and saves weeks of rescheduling headaches.
Missing one item means rescheduling, so double-check before leaving home.
After securing your permit, review Iowa learner's permit restrictions to understand your driving limitations during the instruction period.
The most common document problems are an unacceptable birth certificate, insufficient or duplicate-source residency proof, and a parental-consent form that was not properly witnessed or notarized when required.

"The AI mock tests were surprisingly realistic. The explanations for road signs helped me understand the logic, not just memorize. Passed my permit test on the first try!"
