Laminated Documents: DDS cannot authenticate laminated Social Security cards or birth certificates. At an Atlanta DDS, a family member had added a photo inside the SS card's lamination "for ID purposes." It was rejected immediately - an SSA replacement took 10 days before the applicant could return to DDS.

Wrong Form Version: At Macon, a school stamped the enrollment form using an outdated template. DDS rejected it at intake. The current version was downloaded in the waiting room and resubmitted the same day, but the document problem still cost most of the morning.

Hospital Birth Certificates: Decorative 'souvenir' certificates with footprints are not legal documents and will be rejected. Use a certified copy from the state.

Insufficient Residency Proof: Two documents from the same source (e.g., two bank statements) are often rejected. They must be from different entities.

Complete the online License/ID/Permit Form before your DDS visit and bring the confirmation number. This avoids filling out paper forms at the counter, saving 15-20 minutes during peak hours at busy centers like the DDS in Lawrenceville.

Verify your DDS-1 form's notarization and 30-day validity before your DDS visit. A missing notary stamp or expired form causes immediate rejection, forcing a return trip to the school and a rescheduled DDS visit.

Bring two clearly distinct proofs of residency, like a utility bill and a bank statement, both showing your current address. Using two documents from your bank (e.g., a statement and a credit card offer) is a common reason for rejection and rescheduling.

Do both Georgia residency documents need to show exactly the same address? Both must show your current residential address. DDS also requires the two residency proofs to come from separate sources or separate accounts.

Can one document count for two Georgia DDS document requirements?

A document may appear on more than one accepted-document list, but do not use one document as both required residency proofs. Georgia DDS specifically requires two residency documents from separate sources or separate accounts.

What if you only have one Georgia residency document in your own name?

You still need a second accepted proof of your current Georgia residential address. Depending on what you have, another source may be a school record, postmarked USPS mail, financial statement, utility record, employer document, vehicle record, or another item on the DDS residency list.
