What Forms Does SAAQ Require for the Quebec Class 5 Learner's Licence Application?

There is no single numbered 'permit application form' like a SAAQ permit form number. The SAAQ creates your file digitally at a service centre. You must complete two specific supplementary forms.

Required Forms for the Quebec Learner's Permit

Consent of a Person Having Parental Authority: Mandatory for applicants under 18. Download the PDF from the SAAQ forms page.
Declaration of Illness or Impairment (Form 6224A-30): Completed at the service centre counter to confirm medical fitness to drive.

You cannot fill out a main application form at home. Your application process starts by booking an appointment via SAAQclic and visiting an SAAQ outlet with your documents.

How Is the Quebec SAAQ Learner's Licence Application Completed Step by Step?

The application is a verification process done in-person at an SAAQ service centre. You provide information directly to a clerk who enters it into the system.

What Information Do You Provide During the SAAQ Application?

Personal Information: Provide your full legal name exactly as on your birth certificate and RAMQ health card.
Identity Numbers: Your Quebec Health Insurance Number (RAMQ) is the primary identifier.
Residence Address: Provide your current Quebec address. Update it in SAAQclic before your visit if needed.
Medical Declaration: Answer questions on the Declaration of Illness or Impairment form honestly at the counter.
Voter Registration: Choose if you authorize the SAAQ to share your info with Elections Quebec.
Organ Donation: Indicate your consent for organ donation.

Signature and Parental Consent Requirements

Applicants 18+: You sign the application digitally on the SAAQ signature pad.
? Minors (Under 18): You must sign, and a parent or guardian must sign the separate Consent of a Person Having Parental Authority form. Parents cannot sign the main application for you.

Example: A 17-year-old in Laval arrived at the Henri-Bourassa service centre with an unsigned parental consent form. Their parent was at work and unreachable by phone. The clerk could not process the application, and the appointment was cancelled, requiring rescheduling.

Can the SAAQ Parental Consent Form Be Signed Electronically?

SAAQ's public instructions say the form must be completed and signed but do not state that an electronic signature is accepted for this learner's licence step. To avoid a document rejection, use the current form from SAAQ and bring a signed paper copy unless SAAQ tells you an electronic signature is acceptable.

Can a parent sign the consent form before the SAAQ appointment? Yes. The Class 5 instructions tell minors to present the form already completed and signed by the father, mother, parent or legal guardian.
