There is no single numbered application form. The SAAQ creates your file digitally at the service centre; you complete the required supplementary forms that apply to your situation.
There is no single numbered 'permit application form' like a SAAQ permit form number. The SAAQ creates your file digitally at a service centre. You must complete two specific supplementary forms.
You cannot fill out a main application form at home. Your application process starts by booking an appointment via SAAQclic and visiting an SAAQ outlet with your documents.
The application is a verification process done in-person at an SAAQ service centre. You provide information directly to a clerk who enters it into the system.
Example: A 17-year-old in Laval arrived at the Henri-Bourassa service centre with an unsigned parental consent form. Their parent was at work and unreachable by phone. The clerk could not process the application, and the appointment was cancelled, requiring rescheduling.
SAAQ's public instructions say the form must be completed and signed but do not state that an electronic signature is accepted for this learner's licence step. To avoid a document rejection, use the current form from SAAQ and bring a signed paper copy unless SAAQ tells you an electronic signature is acceptable.
Can a parent sign the consent form before the SAAQ appointment? Yes. The Class 5 instructions tell minors to present the form already completed and signed by the father, mother, parent or legal guardian.
Fix mistakes before submission whenever possible. If an error is found after issuance, SAAQ-caused clerical errors are corrected at no charge; an applicant-caused replacement costs $15.50 for a plastic licence or $5.45 for a paper licence. Full post-issuance correction scenarios belong in the dedicated Errors on Permit article.
For Quebec permit process details, see the Quebec permit guide.
If you spot a mistake on a form like the parental consent before your appointment:
Can you cross out a mistake on a SAAQ form, or do you need a new form? For a clean application, use a fresh copy when you notice a mistake before submission. Do not use correction fluid or leave conflicting information on the form.
Tell the agent about any mistake before you pay or have your photo taken. Verify all information on the screen or printout carefully.
Example: A student in Quebec City verified his information on the clerk's screen and noticed his height was listed as 165 cm instead of 175 cm. He corrected it on the spot for free. If he had signed, he would have needed a replacement licence later for a fee.
Once the licence has been issued, use the dedicated Errors on Permit article for the full correction process.
Form Validity: There is no expiry date on the blank parental consent form. However, the information and signature must be current at the time of your application.
At the Saint-Michel SAAQ, a form downloaded from a regional sub-page was one version behind the current form. The clerk declined it. The applicant returned the next day with the current version from saaq.com.
Use the current form downloaded from the SAAQ website. SAAQ does not publish a blanket rule saying every older version will be accepted, so using the current version avoids a preventable counter problem.
"The AI mock tests were surprisingly realistic. The explanations for road signs helped me understand the logic, not just memorize. Passed my permit test on the first try!"
